Public records request template
A starting point for requesting existing deployment-location records from a Washington public agency.
Hello,
Pursuant to the Washington Public Records Act, RCW 42.56, I request electronic copies of existing records sufficient to identify the planned, proposed, approved, installed, active, relocated, deactivated, removed, rejected, or cancelled locations of Flock Safety cameras operated by or for [Agency Name] from January 1, 2019 through the present.
Please include Flock Falcon ALPR cameras, Condor/Condor PTZ or live-video cameras, and any other fixed Flock camera systems.
Specifically, please provide any existing:
- Flock Deployment Plan, Designated Locations list, or equivalent deployment-location record;
- maps, GIS files, KML/KMZ/CSV files, GPS-coordinate lists, street-address lists, or cross-street lists;
- site surveys, site-selection records, installation work orders or completion records, relocation/reinstallation/removal records, or Existing Infrastructure Implementation Service records;
- permits, right-of-way approvals, pole-attachment approvals, and 811 or utility-locate records associated with camera installations;
- contracts, quotes, order forms, statements of work, equipment lists, or implementation schedules that identify planned or actual camera locations; and
- emails or attachments with Flock Safety or its contractors that contain, attach, or link to deployment maps or camera-location records.
I am requesting existing records, not asking the agency to create a new list. If a deployment map or list is maintained in a Flock or vendor portal rather than downloaded locally, please include any agency-held link, export, screenshot, attachment, site-survey record, or other record sufficient to identify the locations.
Please include historical records even if cameras were later disabled, removed, the contract was cancelled, or a proposed deployment was not ultimately installed.
If the agency has never operated, purchased, piloted, planned, or contracted for Flock Safety cameras and has no responsive deployment records, a brief response confirming that is sufficient.
Electronic delivery is preferred. Please notify me before incurring any fees. If another department or public-records officer is the proper recipient, please forward this request or advise me where it should be directed.

Why the template asks this way
The request asks for existing records rather than requiring an agency to create a new inventory. It also asks for historical, proposed, relocated, and removed deployments so a current-status list does not silently erase earlier evidence.
The resulting records should still be evaluated according to their provenance. A proposed location is not the same thing as an installed camera, and an installation record is not proof of current operation.
